Minor Piercing Consent Requirements

If you are under 18, Florida law requires written, notarized consent from your parent or legal guardian before we can perform a body-piercing service.

Your parent or legal guardian must complete the Minor Piercing Consent Form and sign it in the presence of a notary public. The form must identify the minor and describe the specific piercing being authorized.

Next Steps:

    1. Download and print the Minor Piercing Consent Form.

    2. Complete the form and specify the piercing being authorized. The parent or legal guardian should
      not sign it yet.

    3. The parent or legal guardian must sign the form in the presence of a notary public.

    4. Bring the completed, notarized form to the piercing appointment.

    5. Bring the required identification:

      Under 16: A parent or legal guardian must accompany the minor and present valid government-issued photo ID.

      Ages 16–17: If the parent or legal guardian will not be present, bring a clear copy or photo of the ID belonging to the person who signed the form.

    Important: We cannot perform a minor’s piercing without a properly completed and notarized consent form. Missing consent or identification may require the appointment to be rescheduled.
